Arts & Crafts Classes - Ages 9-16

When you Sign up with any SMAC Programs
This means that you are fully aware or SMAC Rules & Regulations
And are in Complete Agreement with San Marco Art Center Art & Crafts Classes.

Policies and Procedures: SMAC – San Marco Art Center

• Drop off is available 10 minutes before class/camp start time. 
• Pickup is available for 15 minutes after class/camp end time.
• Children need to be picked up promptly.
• Children should dress in a manner conducive to art-making (old t-shirts or smocks are encouraged as spills may happen!)
• Child registration forms (below) are required and can be handed in/completed at first drop-off. 
• Strict sign-in/sign-out procedures are followed. 
• Children should not attend if they are experiencing any symptoms of illness.
• All instructors are background-checked. 
• Children must bring lunch for camps and may bring snacks for both camps and classes.
• SMAC is not responsible for personal property.
• Children are expected to behave in a manner respectful of themselves, each other, instructors, craft materials, and the space.
• SMAC reserves the right to refuse service to any child/family who violates the above-stated policies/procedures.

Acknowledgement of Risk 

I hereby give permission for my child to participate in SMAC - San Marco Art Center programming at ART STUDIO Classes. 

I accept responsibility for the mentioned child who is willingly participating in arts education in which there are certain inherent risks and dangers. These risks may include, but are not limited to, loss/damage of personal property, illness, physical injury, or accidental fatality. 

I understand that although SMAC has taken reasonable precautions to provide a safe environment and qualified staff, it is impossible to guarantee against injury, illness, or loss resulting from participation. I understand that SMAC is not equipped to provide medical intervention. I further recognize that all activities are voluntary and the child may choose not to participate. I agree that I have provided complete and accurate information regarding the child’s age, physical and emotional health relevant to the child’s participation. I understand that failure to properly notify SMAC of the child’s relevant health information may put the child at further risk. I agree to notify SMAC of any changes in my child’s physical and emotional health. 

As the child’s guardian, I expressly agree to accept and assume all risks associated with my child’s participation in the program. I agree to hold SMAC, its employees, contractors, volunteers, directors, and affiliates harmless from any and all claims, actions, suits, proceedings, costs, damages, and liabilities brought as a result of the child’s participation in the program.

I have read the above statement and fully understand its terms, including that I am giving up my right to sue SMAC. I am fully aware of the Rules & Regulations and intend to give SMAC a complete and unconditional release of all liability to the greatest extent allowed by law.
